The treatment of purulent wounds, even in our time, remains very difficult and problematic. The closest attention is paid to the primary treatment of the wound, for which they resort to the most modern physical and chemical means: the use of laser beams, ultrasound, cryotherapy, etc. The prerequisites for the successful treatment of purulent wounds are the use of drains and an active effect on the pathogenic microflora. In practical surgery, the main method of treating such wounds is treatment under a bandage, that is, a certain active factor is introduced into the wound cavity and its edges: disinfectant, phytoncide, depending on the clinical condition of the wound and its size. Antimicrobial factor and dressing provide a favorable course of healing. Practice states that the widespread use of antibiotics, cytostatics, corticosteroids, immunosuppressants over time negatively affects the course of the purulent-inflammatory process, as this greatly reduces the overall reaction of the body, complications such as allergies, candidiasis, etc. Significant resistance of pathogenic microbes develops. to antibiotics, even a wide spectrum of action. The massive, erratic, low-dose use of antibiotics significantly changed the specific role of surgical infection.

Ointments and emulsions (liquid ointments) are more widely used. They do not irritate or damage tissues. But the disadvantage is that they do not adsorb wound exudate and at the same time delay the repair (recovery) processes.

Proteolytic enzymes (trypsin, chymotrepsin, terylitin) are widely used in purulent surgery due to their necrolytic and decongestant properties. Their disadvantage is that they do not provide complete cleansing of the wound, often allergize the body and this complicates the treatment, as well as the fact that when applied topically, they act briefly - no more than 30 minutes. General antibiotic therapy does not suppress the development of microorganisms in the wound, since inflammation disrupts the microcirculation of the damaged area, and microbes settle in necrotic tissue, which has lost contact with the body.

Promising is the test for the therapeutic needs of multicomponent ointments. Their use makes it possible to actively intervene during the purulent wound process and gives a significant therapeutic effect, reducing the healing period by 2 - 2.5 times.

E. Tovstukha has been using herbal remedies for the treatment of purulent wounds for several decades. For this purpose, he proposed an original complex preparation - PROBESAN, which has undergone clinical trials in the Yagotinsky district hospital. Patients with burn disease of 1, 2 and 3 degrees were treated in the surgical department of the Yagotinsky central hospital during 1982 - 1992 (thermal burns in 484 patients, chemical burns in 36). Burns affected 10 to 47% of the skin surface. In order to prevent the occurrence of burn shock and to facilitate the course of it next to the application of generally recognized measures, we irrigate the surfaces with propobesan: during the first 2 days, 2 times every day, during the subsequent ones, once a day. With this treatment, the stage of toxemia bulla is not very pronounced, and the stage of septicotoxemia did not develop at all, therefore, there were no complications. The fact is that when propobesan is used, a bactericidal film is formed on the surface affected by the burn, which prevents the growth of purulent microbes on it, as well as under it. Microbiological plating of the contents of burn wounds in the first 2 days of treatment was sown in 90 cases (17%): blue-green pus bacillus in 50 (55%), pathogenic staphylococcus in 11 (12%), Proteus in 29 (32%) ) However, there was no suppuration, therefore, septicotoxemia did not develop.

The inflammatory response of burn wounds to the administration of propobesane has not developed. Epithelization of burn wounds began from its edges: with a burn of the II degree - on 5-6 days with a burn of the III degree (under the film) - on 10-12 days. When treating burn patients using this method, a significant amount of dressing has been saved. Patients were treated only in an open way. The irrigation method is very simple and the medical staff spends much less time on patient care, while the hospital stay is reduced by 10-20 days. The cost of treatment of one patient according to the above method is reduced by 5-10 times; savings in antibiotics, sulfanilamide drugs, corticosteroids, etc. are achieved.

Propobesan was treated in 500 patients with cervical erosion. Daily for 10-15 days, local cervical irrigation with this drug was used or a swab soaked in it was inserted. 420 patients (84%) were completely cured after 10 days, 60 (12%) after 15 days. In 20 (4%) patients, the erosion area decreased significantly, although it did not reach a complete cure. In these patients, erosion disappeared after repeated treatment with the specified method for 7 days.

PROBESAN - has no analogues in the world pharmacopoeia.
